START WITH THE CURRENT TEST PLAN

The NREMT Paramedic exam at a glance

The National Registry’s Paramedic Certification Examination measures entry-level competency in one computer-adaptive certification examination. The ALS psychomotor examination pathway ended June 30, 2024; check the current handbook for the requirements that apply to your candidate pathway.

110–150items may be administered
3.5 hoursmaximum exam time
20unscored pilot items are included

Use the current version. The National Registry retired the previous ALS psychomotor exam pathway on June 30, 2024. Check the current Paramedic Candidate Handbook and your education program for eligibility, authorization, and scheduling details.

FOLLOW THE OFFICIAL DOMAIN WEIGHTS

NREMT Paramedic exam content domains

The test plan assigns each domain a percentage range. These ranges guide study priorities; they are not a promised item count or a score prediction. Pediatric care is integrated throughout the marked domains.

Clinical domain

Airway, Respiration & Ventilation

8%–12%

Review assessment, pathophysiology, and management across respiratory presentations.

Clinical domain

Cardiology & Resuscitation

10%–14%

Connect assessment findings, cardiovascular pathophysiology, and resuscitation management. NREMT notes that 30% of this domain is allotted to graphical items with ECG rhythm strips.

Clinical domain

Trauma

6%–10%

Organize trauma assessment and treatment decisions around patient findings and scene context.

Clinical domain

Medical, Obstetrics & Gynecology

24%–28%

Review assessment and management across medical, obstetric, gynecologic, and pediatric presentations.

Operations domain

EMS Operations

8%–12%

Revisit leadership, professionalism, communication, documentation, legal standards, vehicles, equipment, and the environment of care.

Cross-cutting domain

Clinical Judgment

34%–38%

Practice recognizing and analyzing cues, defining a hypothesis, generating solutions, taking action, and evaluating results—while communicating and leading within the EMS response.

PRACTICE THE RESPONSE TYPES

How the Paramedic exam presents questions

The computer-adaptive exam can stop once it has enough information to make a pass/fail decision, or continue to the maximum item count or time. Scenario-based questions can use different response formats, so learn the directions before practicing.

Single and multiple response

Choose one response or select the requested number of options. Read the directions closely; partial credit is not awarded.

Interactive item types

Build List, Drag-and-Drop, Options Box, and graphical items may ask you to sequence actions, sort information, or interpret an ECG strip, chart, image, or other visual.

Clinical Judgment scenarios

Scenario questions can ask you to work through changing information from en route, scene, and post-scene phases. Reassess cues as the situation develops.

Unscored pilot items

The test includes 20 unscored pilot items. They are not identified during the exam and do not affect the candidate’s score.

Practice honestly. NREMT’s sample items are examples of interactive item formats, not a prediction of the questions on an individual exam.

TURN THE TEST PLAN INTO A ROUTINE

A practical Paramedic study plan

Use your current course materials, local protocols, and instructor guidance as your clinical references. Then use the test plan to make your review more deliberate.

1

Confirm your exam pathway

Check the current Candidate Handbook, application status, and testing authorization before scheduling.

2

Try the response formats

Use NREMT’s official sample items to get comfortable with graphical, sequencing, and sorting interactions.

3

Rotate through all six domains

Give extra review time to higher-weighted areas while returning to every domain regularly.

4

Explain your clinical reasoning

For each scenario, name the cues, your working hypothesis, the next action, and what finding would make you reassess.

QUICK ANSWERS

NREMT Paramedic exam questions

How many items are on the NREMT Paramedic exam?
The current Paramedic exam plan allows 110 to 150 items. It includes 20 unscored pilot items, which do not affect the candidate’s score.
How long is the Paramedic Certification Examination?
Candidates have up to 3.5 hours. Because it is computer-adaptive, the exam may end earlier if the National Registry can make a pass/fail decision with the required confidence.
What are the six Paramedic exam domains?
Airway, Respiration & Ventilation; Cardiology & Resuscitation; Trauma; Medical/Obstetrics/Gynecology; EMS Operations; and Clinical Judgment. Pediatric care is integrated across marked domains.
What is the Clinical Judgment domain?
It assesses more than recall. The test plan describes recognizing cues, analyzing cues, defining a hypothesis, generating solutions, taking action, and evaluating the result, alongside communication and leadership.
Does the current Paramedic exam include a psychomotor exam?
The current National Registry process uses one certification examination. The ALS psychomotor exam pathway ended on June 30, 2024. Use the current Candidate Handbook for the requirements that apply to you.
What question formats should I expect?
The test plan lists multiple-choice, multiple-response, Build List, Drag-and-Drop, Options Box, and graphical item types. NREMT provides interactive sample items so candidates can practice the response mechanics.
Where can I find official Paramedic sample questions?
NREMT links an interactive Paramedic sample-item experience and a Clinical Judgment sample packet from its ALS examination information page. Those materials help explain the format; they do not reproduce a full live exam.
