MEET THE EXAM

What the NCMHCE measures

The National Clinical Mental Health Counseling Examination measures an entry-level clinical mental health counselor’s ability to apply counseling knowledge and skills to clinical case studies. It is used for counselor licensure in many states, is one of two exam options for the National Certified Counselor (NCC), and fulfills the exam requirement for the Certified Clinical Mental Health Counselor (CCMHC) specialty credential.

CURRENT HANDBOOK · THROUGH JUNE 30, 2027

Know the current case format

For exams before July 1, 2027, NBCC’s state licensure handbook describes 11 case studies. Each case begins with a narrative and unfolds through an intake summary and two subsequent counseling sessions, with 9–15 multiple-choice questions tied to the case.

11 casesOne full case is unscored and helps NBCC evaluate items for future forms.
100 scored questionsScored items are mixed with the unscored case items.
225 minutesTimed exam, plus a scheduled 15-minute break after case five.
255-minute sessionIncludes the agreement, tutorial, exam, and scheduled break.

Each case is organized in three parts: an initial intake summary, counseling session one, and counseling session two. A new narrative segment appears before each question set, so use what the client has already shared and notice how the clinical picture changes.

The current outline lists item weights for five domains. “Areas of Clinical Focus” is represented across the cases rather than assigned a separate item-level percentage.

REVISED VERSION · EFFECTIVE JULY 1, 2027

A new case format and blueprint begin in 2027

NBCC’s new examination specifications are effective July 1, 2027. The cases remain the center of the exam, while the number of cases, time structure, content domains, and scoring information change. Use this outline only for an exam on or after its effective date.

10 case studiesEach case has 9–15 multiple-choice questions.
130–150 questionsIncludes 30–50 field-test items mixed throughout.
225 minutesOptional 15-minute break at the midpoint.
360 scaled scorePassing point on a 100–500 scale; not a percent correct.

Score carefully: the 2027 specification says scaled scores range from 100 to 500 and the passing point is 360. That scaled score does not equal a percentage of questions correct. After submitting the first half, candidates cannot return to those questions.

A PRACTICAL REVIEW MAP

What to review for NCMHCE cases

Use the content outline that matches your date. Build knowledge across the domains, then practice choosing the next clinically appropriate step from the information in a case.

Listen and assess

Intake, risk, and diagnosis

Review

  • Presenting concern, history, context, and strengths
  • Risk to self or others and immediate safety needs
  • Differential diagnosis and co-occurring concerns
  • Assessment gaps and appropriate level of care

Plan with the client

Goals and continuity

Review

  • Prioritize needs and establish measurable goals
  • Connect interventions to client priorities
  • Track progress and revise the plan
  • Consider referrals, transitions, and follow-up

Respond skillfully

Counseling methods

Review

  • Choose approaches that fit the case and client
  • Use listening, reflection, and collaboration
  • Adapt to cultural and developmental context
  • Consider group, family, and systems dynamics

Practice responsibly

Ethics and professional judgment

Review

  • Confidentiality, informed consent, and records
  • Scope, consultation, supervision, and referral
  • Boundaries and competing responsibilities
  • Self-awareness, access, and respect for clients

Study the case in sequence. Later questions may depend on what changed since intake. Keep a brief timeline of symptoms, risk, client goals, and the counseling response as you practice.

PRACTICE CLINICAL DECISIONS

A repeatable approach to each case

The NCMHCE asks you to use case information to identify, evaluate, and respond to clinical concerns. Practice reasoning from the narrative instead of memorizing isolated facts.

1

Frame the concern

Summarize the client’s presenting concern, relevant context, strengths, and what you still need to know.

2

Check safety and assessment

Notice risk indicators, urgency, diagnosis clues, and whether more assessment or a higher level of care is needed.

3

Choose the next response

Select an ethical, evidence-informed action that fits the client’s goals, stage of care, and circumstances.

4

Follow the change

As the case advances, revisit your working understanding, evaluate progress, and adjust the plan when new details appear.

Use original practice cases. Protect examination security. Do not seek, reproduce, or share live or recalled exam content.

A ROUTINE YOU CAN ADAPT

Build a focused study plan

Start with a timed case set and use your errors to decide what to review next. Give extra practice to the areas that are both high-weight and difficult for you.

1

Confirm your exam version

Write down your test date, then download the matching NBCC handbook and content outline.

2

Take a baseline case set

Record uncertain answers and tag misses by assessment, ethics, planning, intervention, or counselor judgment.

3

Practice one decision skill

Review the concept, explain your reasoning, and apply it to a new case vignette with different details.

4

Return to mixed cases

Practice timed cases in sequence, then review why each answer fits the evidence available at that point.

Make the version switch deliberate. The 2027 content outline changes the domain names and weights. Keep your notes labeled by exam version.
