NBCOT OTR Study Center & Exam Guide
Preparing for the Occupational Therapist Registered (OTR) certification exam? Start with NBCOT’s current four-domain outline and scenario format, then build a steady plan around occupational performance, client priorities, intervention choices, and professional practice.
MEET THE EXAM
What the NBCOT OTR exam measures
NBCOT uses the OTR certification exam to assess entry-level occupational therapy knowledge and clinical reasoning. The outline follows the occupational therapy process: understand the person and context, interpret findings and set priorities, select and manage interventions, and uphold safe, evidence-informed professional practice.
The OTR exam is for occupational therapists. NBCOT’s COTA exam is a different certification exam with its own item total and content outline.
CURRENT NBCOT EXAM INFORMATION
How the OTR exam works
Single-response items
Each item has three or four choices. Select the one best response for the question.
Multi-select scenarios
An opening scene is followed by four related items. For each item, choose three best responses from six options.
Screen tools and pacing
You can highlight details, strike out options, flag items, and review answers. The tutorial is optional; tutorial time does not count toward the four-hour exam clock.
Score and time
NBCOT reports a criterion-referenced scaled score from 300 to 600. A score of 450 passes. The pass decision uses total exam performance; there is no separate domain passing score. The appointment is about four hours and 45 minutes. You may take breaks, but the exam clock continues.
There is no deduction for incorrect answer choices. For multi-select items, NBCOT awards credit for correct response options, so consider each option against the case rather than trying to guess an all-or-nothing pattern.
BEFORE YOU APPLY
Check OTR exam eligibility
U.S.-educated candidates
NBCOT requires graduation with an entry-level occupational therapy degree from an ACOTE-accredited OT program.
Internationally educated candidates
Candidates whose entry-level OT education was outside the United States must use NBCOT’s OT Eligibility Determination (OTED) process before applying for the OTR exam.
Plan through MyNBCOT
Use your MyNBCOT account for the application, Authorization to Test, scheduling, current fees, and available testing dates. Confirm the current handbook before applying.
NBCOT certification and state licensure are related but separate. Check the licensing board in the jurisdiction where you plan to practice for additional requirements.
YOUR CONTENT MAP · CURRENT NBCOT OUTLINE
The four domains on the OTR blueprint
NBCOT’s 2022 OTR Examination Content Outline assigns the following proportions to the exam. Use the weights to guide study time, while reviewing all four domains because the outline describes a connected occupational therapy process.
1. Evaluation and Assessment
Gather information about the person, their lived experience, body functions, roles, needs, and performance context. Review occupational profiles, screenings, assessments, and activity analysis.
2. Analysis, Interpretation, and Planning
Synthesize quantitative and qualitative findings, identify eligibility and priorities, collaborate on a client-centered plan, and monitor or adjust it using clinical reasoning.
3. Select and Manage Interventions
Choose preparatory methods and occupation-based interventions; support daily activities, sensory and motor function, orthotic use, assistive technology, mobility, and accessible environments.
4. Competency and Practice Management
Apply evidence-based practice, risk management, infection prevention, laws and regulations, reimbursement requirements, ethical standards, and ongoing competency development.
Percentages reflect the current outline and may be updated by NBCOT. The outline lists tasks and knowledge statements within each domain; see the official source for the complete detail.
PICK A DIRECTION
Choose a study route that fits your gaps
Start with an honest baseline. For each missed, guessed, or slow answer, note the domain and why it was difficult: missing knowledge, confusing context, weak prioritization, intervention choice, or professional practice.
Read the blueprint
Turn each domain and task into a checklist of concepts you can explain.
Connect to occupation
Ask how the person, activity demands, environment, and performance fit together.
Practice scenarios
Use the case details for each of the four related items; assess all six options independently.
Review the reasoning
Explain why each selected choice fits the case and why other options do not.
When two domains are equally weak, give more scheduled practice to Select and Manage Interventions because it carries the largest outline weight, then return to the other areas.
A ROUTINE YOU CAN ADAPT
A four-week OTR study plan
Set realistic daily sessions and protect time to review errors. NBCOT recommends using the official outline and preparation tools to identify weak areas and shape a personal study plan.
Read the outline, complete a self-assessment, and map results to the four domain weights.
Review occupational profiles, activity analysis, assessment findings, client priorities, and collaborative plan development.
Rotate through occupation-based intervention choices, assistive technology, safety, evidence, and professional requirements.
Take a timed practice exam, revisit patterns in the feedback, review scenario sets, and prepare test-day logistics.
After practice, revisit explanations and outline statements rather than memorizing questions. NBCOT exam items and its proprietary study material are protected; do not reproduce or share them.
