MEET THE EXAM
What the NBCOT COTA exam measures
The NBCOT COTA certification exam measures the knowledge and judgment needed for entry-level occupational therapy assistant practice. The outline follows three connected responsibilities: collaborate and gather information, select and implement interventions under OTR supervision, and uphold professional standards.
COTA is the occupational therapy assistant exam. Its blueprint, eligibility, and item formats differ from the OTR exam.
CURRENT NBCOT EXAM INFORMATION
How the COTA exam works
Single-response items
Each question has three or four answer choices. Select the one best response; incorrect choices do not reduce your score.
Six-option multi-select items
Each item has six choices. Select the three correct responses. NBCOT awards credit for each correct option selected, with no deduction for incorrect options.
Screen tools and pacing
Items appear one at a time. You can highlight details, strike out options, flag questions, and review responses. The optional tutorial does not use exam time.
Score and time
NBCOT reports a criterion-referenced scaled score from 300 to 600. A score of 450 passes. The pass decision uses total exam performance; there is no separate passing score for an individual domain. You have four hours of exam time and the full appointment is about four hours and 45 minutes.
You may take breaks, but the exam clock continues. COTA multi-select items are standalone; they are not presented as related questions within a scenario set.
BEFORE YOU APPLY
Check COTA exam eligibility
U.S.-educated candidates
NBCOT requires an entry-level occupational therapy assistant degree from an ACOTE-accredited OTA program.
International education
NBCOT states that an internationally educated occupational therapist degree does not qualify a candidate for the COTA exam. The COTA pathway requires an OTA degree from an accredited U.S. OTA school.

YOUR CONTENT MAP · CURRENT NBCOT OUTLINE
The three domains on the COTA blueprint
NBCOT’s 2022 COTA Examination Content Outline assigns these proportions to the exam. Use the weights to set study priorities, while reviewing all three domains and the tasks within them.
1. Collaborate and Gather Information
Support the OTR by gathering information about the client, occupational performance, needs and context; collaborate with clients and relevant others; monitor progress and communicate changes.
2. Select and Implement Interventions
Carry out the intervention plan under OTR supervision. Review occupation-based activities, preparatory techniques, adaptation, safety, client participation, response to intervention, and appropriate modifications.
3. Uphold Professional Standards and Responsibilities
Apply ethical and legal requirements, documentation practices, safety and risk management, communication, supervision, service delivery policies, and professional development.
Domain weights and names follow the 2022 COTA outline. Confirm the latest outline on NBCOT’s website before scheduling.
PICK A DIRECTION
Choose a study route that fits your gaps
Start with a self-assessment or practice set. For each missed, guessed, or slow item, note the domain and the reason: missing knowledge, unclear context, intervention selection, safety, or professional judgment.
Read the blueprint
Turn each domain task into a checklist of ideas you can explain.
Think through the context
Connect the client’s needs, activity demands, environment, and intervention plan.
Practice both item styles
Choose one best response or assess each multi-select option on its own.
Review your reasoning
Explain why each response fits and what information you still need to review.
When prioritizing time, spend extra practice on Select and Implement Interventions because it carries the largest blueprint weight. Keep all three domains in your rotation.
A ROUTINE YOU CAN ADAPT
A four-week COTA study plan
Choose realistic study blocks and reserve time to review practice results. NBCOT’s self-assessment and outline can help you identify where to begin.
Read the outline, complete the self-assessment, and map strengths and gaps to the three domain weights.
Review client information, occupational profiles, performance context, communication, and progress monitoring.
Practice selecting and carrying out interventions under supervision, including activity adaptation and safety.
Complete timed practice, review score feedback, revisit weak tasks, and confirm test-day logistics.
Use practice material to study the reasoning and concepts. NBCOT exam items and proprietary study material are protected; do not reproduce or share them.
