HESI Exit Exam Study Center

Prepare for the HESI Exit Exam with a practical review map, score guidance, and a focused study plan. This center covers RN and PN exit exams. Your nursing program sets the required version, testing conditions, and its own passing standard.

A PROGRAM-ASSIGNED READINESS ASSESSMENT

Know which HESI exam you are taking

The HESI Exit Exam is a nursing-program assessment used near the end of a course of study to help evaluate readiness for the NCLEX. There are RN and PN forms. Elsevier also offers many course-specific specialty exams, and HESI A2 is a separate admissions assessment. Check your course or program instructions for the exact exam name and platform.

RN or PNConfirm the licensure pathway and form assigned by your program.
Version can varyHESI testing may run in different environments and exam versions.
Score up to 1500Elsevier describes HESI Exit and specialty exam scores on a typical 0–1500 scale.
School-set standardPrograms choose their own course, remediation, and progression policies.

Before you study: ask your instructor which Exit exam (RN or PN), version, time limit, retake policy, and score threshold apply to you. Testing settings can be set by the program and platform.

A STUDY MAP, NOT A PUBLIC HESI WEIGHTING

Review core nursing and clinical judgment skills

Elsevier says HESI Exit exams align with the NCLEX test plan and include clinical judgment. Mometrix’s HESI Exit guide organizes review around the familiar Client Needs areas below. I found no single current public candidate blueprint with fixed topic weights for every RN and PN form, so treat these as a broad review checklist and use your HESI results and school remediation report to prioritize.

Client Needs

Management and coordinated care

RN & PN focus

Set priorities, delegate within role and policy, coordinate care, communicate changes, and support safe transitions.

  • Who should be seen first?
  • What can be delegated, and what needs follow-up?
  • What belongs in a handoff or escalation?

Client Needs

Safety and infection prevention

RN & PN focus

Reduce risks for clients and staff while applying precautions, safe equipment use, and error-prevention practices.

  • Standard and transmission-based precautions
  • Fall, aspiration, and environmental safety
  • Recognize and report hazards or changes

Client Needs

Health promotion and maintenance

RN & PN focus

Connect age, development, prevention, and teaching to the client’s health needs and readiness to learn.

  • Screening and preventive care
  • Growth, development, and aging
  • Teaching, learning, and follow-up

Client Needs

Psychosocial integrity

RN & PN focus

Use therapeutic communication and respectful care while responding to mental-health, coping, and social needs.

  • Therapeutic response and boundaries
  • Stress, grief, and crisis support
  • Culture, privacy, and client preferences

Client Needs

Basic care and comfort

RN & PN focus

Plan and evaluate everyday care that supports function, comfort, hygiene, mobility, and nutrition.

  • Activity, positioning, and skin care
  • Nutrition, elimination, and rest
  • Pain, comfort, and assistive devices

Client Needs

Pharmacology and parenteral therapies

RN & PN focus

Apply safe medication principles, calculations, monitoring, and teaching within your role and scope.

  • Rights and checks before administration
  • High-alert medications and adverse effects
  • Dosage, routes, and response monitoring

Client Needs

Reduction of risk potential

RN & PN focus

Interpret trends and test results to identify complications early and determine what needs prompt action.

  • Laboratory values and diagnostic findings
  • Pre- and post-procedure monitoring
  • Changes from baseline and complication risk

Client Needs

Physiological adaptation

RN & PN focus

Recognize unstable conditions and connect pathophysiology, assessment findings, and appropriate response.

  • Acute and chronic condition management
  • Respiratory, cardiac, neurologic, and endocrine changes
  • Prioritize cues, interventions, and reassessment

Clinical judgment runs through the topics. For each scenario, practice recognizing important cues, analyzing what they mean, prioritizing likely problems, choosing safe actions, and evaluating the response. The 2026 NCLEX test plans can guide current RN or PN review, but they are not a substitute for your school’s HESI outline.

UNDERSTAND THE SCORE IN CONTEXT

HESI Exit Exam scoring and benchmarks

HESI score is not percent correct

Elsevier describes the HESI score as a proprietary measure, typically from 0 to 1500, that considers item difficulty and performance. The conversion score is a separate calculation some programs use for grades.

850 is an Elsevier benchmark

Elsevier labels 850 an “Acceptable” content-mastery benchmark and reports research linking scores at or above 850 with first-attempt NCLEX success. It is not a universal passing score or a guarantee of an individual outcome.

Your program determines passing

Elsevier says faculty and administrators set school benchmarks and policies. Ask how your program uses the score, whether it is part of a course grade, and what happens if you miss its target.

Use item feedback to remediate

Review the score report and assigned learning plan. Elsevier describes partial credit for some next-generation and multiple-response item types, so focus on the report’s feedback rather than trying to reverse-engineer a raw percentage.

Elsevier’s published score research reports outcomes for groups of students. It cannot predict any one student’s result, and an institution may set a higher or lower requirement for its own program.

A PRACTICAL REVIEW ROUTINE

Build a focused HESI Exit study plan

Start with your assigned exam and remediation results. Revisit nursing concepts in context, then practice using clinical judgment instead of memorizing isolated answer patterns.

1

Confirm the requirements

Write down RN or PN, exam version, date, testing platform, time limit, school target, and retake or remediation policy.

2

Find your weak areas

Use your course exams, HESI feedback, and an authorized practice set. Sort misses by client need, condition, medication safety, and reasoning step.

3

Study with a clinical loop

For each case, identify the cue, explain the risk, set a priority, choose an action, and decide what result would show improvement.

4

Practice and review deliberately

Use the time and tools assigned by your program. Review every missed or guessed item, explain why alternatives are less safe, and revisit the concept later.

Protect exam security. Do not use recalled or leaked live exam questions. Use authorized learning resources, concept review, and original practice items.

QUICK ANSWERS

HESI Exit Exam FAQs

What is the HESI Exit Exam?
It is a nursing program assessment used near the end of study to evaluate readiness for the NCLEX and identify areas for review. Programs may use the results in different ways.
Is HESI Exit the same as HESI A2?
No. HESI A2 is an admission assessment for applicants. HESI Exit is used in nursing programs near the end of study. Use resources for the exam your school assigned.
Is there an RN and a PN HESI Exit Exam?
Yes. HESI provides RN and PN exit assessments. Confirm which form and testing version your program requires.
How many questions are on HESI Exit and how long is it?
Question count and testing settings can depend on the assigned exam version and platform. Ask your instructor or testing coordinator for your exact item count, time limit, break rules, and review settings.
What is a passing HESI Exit score?
There is no single passing score used by every nursing program. Elsevier describes 850 as an acceptable content-mastery benchmark, while each program sets its own threshold and policy.
Does an 850 guarantee that I will pass the NCLEX?
No. Elsevier reports group-level research associating scores of 850 or higher with strong first-attempt NCLEX outcomes. A benchmark is not a guarantee for an individual student.
What should I study for HESI Exit?
Use your assigned HESI score report and program guidance first. A broad study map includes care coordination, safety, health promotion, psychosocial care, comfort, pharmacology, risk reduction, and physiological adaptation, with clinical judgment across each area.
Is the Mometrix HESI guide an official Elsevier product?
No. Mometrix identifies its HESI guide as an independent product and states that Elsevier was not involved in producing or endorsing it. It can be an optional study aid; official exam information comes from Elsevier and your program.
