NCLEX-PN — Original 85-Question Practice Test Reference: NCSBN 2026–2029 PN test plan. English. Source-only printed practice adaptation. Allow a 300-minute session including orientation and breaks. The actual NCLEX-PN is adaptive and may contain 85–150 questions; this practice form uses the 85-question minimum. It includes 52 content-domain items, three six-question clinical cases, and 15 additional learning items. All practice items receive teaching points, including the 15 supplemental items. These are original questions and do not reproduce a secure exam. Work in order. Cover later case updates until reaching them and avoid returning to a completed item after advancing. Every case question supplies the information needed at that point; the answer to an earlier question is never required. A basic calculator and blank working space are permitted here as substitutes for the supplied exam tools. Do not read the answer and scoring companion until finished. Unless an age is stated, the client is an adult. Preferences, identity and beliefs described in a question are that client's stated preferences. The practical/vocational nurse collects focused data, reinforces teaching, contributes to the plan and collaborates with the RN and other clinicians. Follow the legal permissions, facility rules, orders and emergency capabilities explicitly supplied in each scenario. Do not infer authority to independently prescribe or make a medical diagnosis. Response instructions: - Ordinary single-choice items: select exactly one letter. - Select-all or printed highlight items: select every applicable listed option. Highlight approximations permit selecting only the listed phrases; no other text is selectable. - Matrix items: select exactly one option in each row. Letters continue sequentially across the whole item instead of restarting with A. - Printed drop-down/rationale items: select one option in each named row to complete the statement. The two selections form a linked explanation. - Bow-tie approximation: follow the separate limits: two action tokens, one condition and two monitoring tokens. Use the letters actually printed in each group. - Trend questions: interpret the supplied successive findings and select one conclusion. TREND names the reasoning task, not a claim of a separate native response format. - Numeric items: enter the requested number and unit. Do not round unless instructed. Scoring for learning: - Single-choice and numeric entries:1 point for the specified correct answer,0 otherwise. - Matrix:1 point per correct row. Multiple selections or no selection in a one-response row earn 0 for that row in this printed adaptation. - Select-all/highlight: add 1 for each correct selection and subtract 1 for each incorrect selection, with a floor of 0 for that item. - Linked drop-down rationale pairs: this practice form awards 1 point only if both linked rows are correct. A partially correct pair earns 0. This is a declared teaching-point convention and does not establish a secure NCLEX calibration. - Bow-tie: up to 5 points, one per correct target. Either ordering of the two correct action tokens or the two correct monitoring tokens is accepted. Selecting more than the specified number makes that group invalid in this paper exercise; the native interface would limit selections. The maximum teaching score is 131 points. Use mistakes to identify topics for further study. Do not convert these points into the NCLEX ability scale, an official pass/fail result or a probability of passing.
